I have several lines on my QRZ logbook that have dots within dots--telling me that the other station has also asked for confirmation. All the info is correct. Under these conditions, wouldn't that simply result in a star for confirmation? What am I missing?

If one or both of you made a mistake while logging as in wrong band, wrong mode or perhaps wrong time (e.g. logging local time instead of UTC) then the QSO won't be automatically confirmed even if both parties have requested confirmation. IIRC, the QSO times have to match to plus or minus half an hour to be automatically confirmed. You could check your logged data and then check with the other ham to see if there's some kind of mismatch problem.
