Just getting back in the game and I came up with this:

This looks like you stole someone else's design and put some stuff on top of it...

I recommend using a transparent ARRL logo so there is no white area and moving it to the left. I assume you will be using a silver or white ink pen for the fill-in-the-blanks area.
Also, bring your callsign down some so it doesn't block the location text. I would also use a light color on the callsign and name to give it more contrast and readability.

A good start, just some tuning is needed. As is ever thus with this hobby.
Show what your QSL card looks like
Look at the qsl thread, stickied at the top. All the qsl cards you can stand.
