Contact info on a Special event station

    In trying to design a nice certificate of contact for a special event station.
    What would be a good design for contact information?
    A little about the Event station the call used and all the logos involved ,, I know. but how about info of the event station operator?

    Just what should be on there about the station making the contact?
    CALL
    NAME
    TIME/DATE
    seems so Spartan, but I guess simple is best?>
    Looking for suggestions and ideas from some experienced Ham's .

    I see almost 100% of the time the usual log info. Just add either frequency or band to your proposed information and you will make everyone happy.

    I have seen where special event stations QSL cards have a place for the operator's name to be put, but I bet they are using special logging software that will transfer that information into a QSL card printing program.
