I've Google'd to find exactly what model Cushcraft this antenna is and so far I'm only sure that it's a Cushcraft.
The ham I got it from told me it was from a pair that came in a set for an array, which I think those arrays are referred to as "boomers." So what you see and get is just ONE of the two, not two yagis.
I have the spreader pole/"stacking boom" too. I can throw it in free plus the shipping.
The closest match I see for what it looks like and probably performs close to is:
